Update: resolved. I resubmitted with an explanation in the review notes (App Store Promotion not enabled, no offer codes or win-back offers, so the images aren't displayed anywhere), and the app and both subscriptions were approved without the images needing to be changed. Posting in case it helps anyone else stuck on the same 2.3.2 issue with locked subscription images.
